How do I make a JUA Student Health Claim?
To make a claim in the event of an incident, parents and guardians should do the following:

Download the Claim form
Download, print and complete your Claim Form. Note that the Medical Attendants Statement on page 2 must be completed by the Medical Attendant.

Mail the completed Claim form to us
